A thorough analysis of an accidental death of a Canadian has just been released. Like all accidents there are multiple causes. My analysis is that the most strategic thing to change to make the world a safer place is to improve computer science and its effectiveness in the world. HCI also failed in a major way. This requires a major cultural shift, as well as better education, standards and so forth.
